What are the benefits of membership in Primary Care Plus?
Primary Care Plus is a healthcare model that emphasizes personalized and patient-focused care, prioritizing direct access to our primary care providers and their team.
Some of the benefits of the program are:
- 24/7 Access to your provider or there care team
- Same Day or Next Day appointments with your provider
- A dedicated Care Coordinator to help with Prescriptions, Refills, Scheduling of Referral Appointments, Labs and Imagining.
- Office visits that start on time with little to no waiting
- Guaranteed 30 minute appointments with your provider face to face.
What is the Annual Membership Fee?
The annual membership fee is set at $365.00 which allows our providers to maintain a reduced patient panel and offer services not covered by insurance.
Is my annual membership fee covered by my insurance?
While the annual membership fee is not covered by insurance, in some circumstances, a flexible health spending account (FSA) can be used to pay for the membership fee. Be sure to check with your FSA administrator to confirm that your membership fee is an eligible expense.
Do I have to pay the Annual membership fee upfront?
No, you can choose to pay the annual membership fee bi-yearly in two equal installments. Those wishing to pay the annual membership fee upfront will be given a 10% discount.
If I wish to cancel my Membership with Primary Care Plus will I be given a refund on the membership fee.
Annual Membership Fees are broken into yearly cycles, if you have seen your provider anytime in that calendar year the fee is not refundable. If you have not seen your provider and you wish to cancel your membership, the fee will be prorated. Please keep in mind that if you have been seen by the provider and are on a bi yearly billing cycle you will be responsible for any outstanding balance. All other refunds are at the discretion of the practice and provider.
How does Primary Care Plus work with my Health Care Insurance?
Primary Care Plus membership is not health insurance. We work in conjunction with traditional health insurance plans. Primary Care Plus accepts most health insurance plans, and bills your office visits like a normal physician practice. Members are still responsible for copays, coinsurance and deductibles which are due at time of service.
Do you accept members who have Medicare?
Yes, we do accept members who have Medicare. All of our Providers in the Primary Care Plus program are participating Medicare providers.
Does my husband, wife or partner (family plan) allow for a discount on the annual membership fee?
No, unfortunately each member in the program pays the same fee without a discount. We do not see children in this program, only adults over the age of 18.
Can I join Primary Care Plus if I don’t have health insurance?
Yes, we offer competitive self pay pricing for office visits and testing due at time of visit. Please see our Self Pay Pricing List on our website.

In his practice, Dr. Zhang is guided by a simple philosophy: the best medical care combines solid scientific evidence in tandem with the individualized goals/values of each patient. He works collaboratively with patients to design individualized care plans that are both effective and sustainable, empowering them to take an active role in their health.
While comfortable treating patients across age/gender/medical history —from children to older adults — Dr. Zhang has a particular passion for men’s health. He has a strong interest in lifestyle medicine, exercise, weight management, and addressing conditions commonly faced by men. His goal is to help patients build long-term habits that improve not just their health metrics, but their overall quality of life.
Whether it’s preventive screenings, chronic disease management, or personalized guidance on fitness and nutrition, Dr. Zhang strives to be the doctor for ‘patients who hate going to the doctor’ and to create a supportive environment where you are heard, respected, and motivated.
Outside of medicine, you can find Dr. Zhang trialing new recipes on the Blackstone griddle, spreading Liberty in Helldivers 2, or trying to convince his patients to join him for fishing along the Delaware Bay.

I came to Lewes to practice family medicine in 2006 after visiting my parents who had retired here and becoming smitten with the region and the community. My journey prior included an undergraduate engineering degree and medical training at the University of Pennsylvania (now the Perelman) School of Medicine and Thomas Jefferson University Family Medicine residency program, the combination to which I attribute my problem-solving and diagnostic skills. I then spent over 6 years in rural Iowa during which, out of necessity, I acquired a wide range of general medicine skills and practical knowledge. My time there included many hours as the only physician present in the ER or ICU setting.
I am so fortunate to have had the training and experience to be your advocate in health. I regard my relationship with a patient as a partnership that promotes wellness and mitigates risk factors for disease. Of course I don’t like to find ailments such as heart and lung disease and cancer; however, I’m particularly passionate about finding these conditions early so that my patients will have the best possible outcomes. I have years of experience in managing hypertension, diabetes, high cholesterol and other adult medicine challenges.

His professional journey has included hands-on clinical experience across multiple specialties—Neurology, Pathology, Internal Medicine, and Pediatrics. He has also contributed to research in Critical Care, Cardiology, Pulmonary Medicine, and PTSD with several abstracts presented and submitted at major conferences.
Dr. Kouzov’s interest in holistic health led him to focusing on proactive wellness strategies into expanding the healthy lifestyle approach to medicine. Following the latest medical and scientific research to help his patients live longer, healthier lives. He remains committed to lifestyle medicine and human-centered care.
Fluent in English and Bulgarian, Dr. Kouzov is passionate about lifestyle medicine, outdoor sports, and building strong relationships with patients to improve both mental and physical longevity.
Outside of medicine, Dr. Kouzov enjoys an active lifestyle including snowboarding, biking, swimming, and running. He also has a passion for photography and travel. He cherishes spending time with his family and their two cats, Bowie and Iris.

The world of medicine was quickly turned upside down by the COVID-19 pandemic causing supply shortages, staffing deficits, and ever-changing medical guidelines that forever shaped Dallas’s ability to adapt and overcome the trials of healthcare. Dallas devoted her time as Charge Nurse to both the General Surgery Unit and COVID-19 Unit, before relocating to the Dallas/Fort Worth (DFW) Metroplex where she ultimately returned to graduate school in January 2022. While in graduate school, Dallas worked full-time as a pediatric home health care RN, taking care of medically complex children in the comfort of their own homes. She graduated from The University of Texas Health Science Center at Houston, earning her Master’s Degree of Nursing in August 2023. Dallas is board-certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner (FNP).
She is a life-long learner and avid enthusiast for holistic care and shared decision making. Dallas put her passion for preventative medicine into action by practicing as a Primary Care Provider for 12 months in the booming DFW Metroplex before relocating back to her Delaware roots in 2024, where she has joined the Atracare team.
When not serving the needs of others, Dallas can be found with family at your local farmer’s market, beaches, or out and about with her two pups. Dallas keeps family values at her core, and carries that passion into the care she provides as an FNP.
Dallas believes wholeheartedly in the patient/provider relationship, and encourages all patients to remember she is simply here to walk alongside you in your healthcare journey! You lead the way.

My practice is largely influenced by my decade-long experience as an RN where I worked in both an acute inpatient and home health care setting. Each employment opportunity provided me with valuable insight into the many facets of healthcare, including the barriers of self-managing acute and chronic illnesses.
When pursuing my Doctor of Nursing Practice degree, I took the opportunity to delve deeper into these barriers with a primary focus on social determinants of health. I have incorporated this knowledge into a holistic lens that expands my ability to partner with patients and guide them towards optimal health and wellness.
